The pain for a calcaneal stress fracture can be anywhere in the calcaneus or heel bone. The most common site for the pain is when you squeeze both sides of the heel bone and the pain can be quite exquisite and that is in the bone just below the insertion of the Achilles tendon. Due to the location this can get confused with insertional Achilles tendinopathy. This location probably account for 2/3rd to 3/4’s of the cases of a calcaneal stress fracture. About a 1/4 are further forward on the heel bone on the middle. A few are in the from part of the calcaneus under the ankle joint.
